In order to verify your account, you must be willing to provide some (or all - please see chart below) of the following documentation: (you will need 2)

• A photocopy of your photo ID
• A copy of a current utility bill with your name and address on it.
• A current bank statement with your name and address on it.

Please note: You may blackout SS numbers and drivers license numbers but your name and address must be clear.

There is a $5.00 charge per year for this service. For this verification, we accept only personal/business checks or US Postal money orders. All funds must be in US dollars and drawn on a US bank.

Please refer to the chart below for information relating to sending a check or a money order and the documentation you will need for either one.

Checks or money orders are to be made out for $5.00 and mailed with the form below. Payable to:

Any forms sent to Ebanned incomplete, without the proper documentation or a money order that is not issued by the US Postal Service will be returned to the sender.
